Customers are prompted to fill out both the Bill To and Ship To fileds at checkout even though the Billing Information is marked as Optional in the admin.
When the Outside US option is removed from the States list in the admin it causes the Bill To fields to be required.
There has to be an option in the States list with a Null code. If the customer has removed this option because they do not wish to ship internationally, they can add an option back in that says "US orders only" or something similar. They will need to leave the Code field blank.
There is a bug in some older versions of Miva Merchant that prevent you from adding items to the State list without a code. If this happens to you, you will need to upgrade to the most recent version of Miva Merchant before you will be able to correct this issue.
